Faith Formation

Welcome to Faith Formation! We are eager to get to know you and share the Catholic faith with the children of our parish community.

During the year we provide a a mandated age appropriate program required by the Diocese of Fall River. It is called the Circle of Grace (COG). The Circle of Grace teaches children and youth personal safety skills including recognizing physical, emotional and spiritual boundaries, identifying trusted adults and taking action if boundary violations occur.

Only return the COG OPT-OUT form if you do not want your child to participate in the lesson(s) or if you want to teach the lesson to your child.

To start your children in Faith Formation classes you may either download or obtain from St. Mary’s Rectory the following forms:

    1. Registration Form
    2. Circle of Grace OPT-OUT
    3. Circle of Grace Disclosure

Please fill out the registration form completely (Only return the COG OPT-OUT form if you do not want your child to participate in the lesson(s) or if you want to teach the lesson to your child) and return them to the rectory with all payments  for registration, retreats and fees.

Remember to write legibly when completing forms and reach out to the director if your contact information changes. We are unable to contact you regarding emergencies with your child, cancellations etc, if your phone number or email is incorrect.
